The cover art lists the date as October 23, 1968 - however, this not appear to be a valid concert date, so it's up in the air as to the date and venue of this recording. 

This *could* be October, 13th, 1968 - Coliseum, Chicago, IL, US (Thanks RW). 

As it is an awful audience recording, it is best avoided by anyone but the most avid collectors.
